License Renewal:
Full practitioner licenses are renewed annually; the effective period is from February 1st of one year to January 31st of the following year. Renewal applications will be mailed in early December, 2009, for the renewal due January 31, 2010.

As you complete the section of the renewal form that asks you to list persons under your clinical supervision, please refer to the summary of supervision requirements, and note the difference between supervision of unlicensed persons, and supervision of a Temporary license holder. Only a Temporary Licensee may perform initial evaluation and final fitting and dispensation activities under "distance" supervision. Please note as well the benefits offered by Temporary licensure to those on track to gain full licensure in the profession. Progression of a license from Temporary to Full now only requires submission of documentation reflecting successful exam and/or completion of supervision - no new application, no new fee!

We cannot accept renewal applications by fax, as payment must accompany the form. We expect to offer online renewal during next year's cycle.

Late renewals incur a $150 penalty on top of the renewal fee; the penalty fee requirement is established by statute and rule 4779-12-01, and thus is mandatory and cannot be administratively waived. OAC Rule 4779-8-01 provides that a non-renewed license may be placed on "lapsed status" by the Board, and any licensee who fails to renew and continues to practice more than 30 days past expiration date may be subject to enforcement action
